As for falloff: It often helps to draw into the image. If you consider it as a means for styling, falloff gets welcome. With falloff, we look at a picture, without, we look through a hole in a wall. As for technical measures, get a medium format lens so that you can scan with the sweet spot of the lens. Please note that a better lens shows more of the original image’s shortcomings.

Flat image: Convert with low pre-saturation and a preset that reduces overall contrast. For colour negatives, I use a preset that I set for soft shadows and lights and white and black points to -10 each. This makes room for microcontrast/structure tweaks.
